"It actually works!" update.  This version has bugfixes that prevented it from working with ASP.NET Identity.
* Creating users and roles now sets the Id on the provided instance.  Inheritors have the option of overriding the default algorithm (Guid as string :/)  
* Refactored AzureTableStore to require storage providers supply a table name when calling.  This allows each provider to specify its own table, which they all do (except for the password store).  
* Added a method for updating the user partition key based on user name.  Inheritors can override the default logic, which is the same as the last version.
